WHAT YOU NEED: The voluntary petition asks for personal information such as your name, address, SSN, phone number, what chapter of bankruptcy you are filing, whether your debts are personal or business related, and then has you check boxes to estimate how many creditors you have, how much debt you have, and how much you have in assets.

The voluntary bankruptcy petition also asks about other bankruptcy cases that may still be open or pending and if there are any issues with your landlord.

You will also need a list of your 20 largest unsecured creditors that have claims.
